- Fee covers: MIVA League Dues, MIVA Championship Tournament, BTMVA Round Robin, BTMVA Championship, Conference Awards and conference administrative expenses. The cost of the BTMVA Awards Banquet will be billed separately.

- Fee covers: MIVA League Dues, MIVA Championship Tournament, M10VC Playdays and Championship, and Conference Awards.

- Fee covers: MIVA League Dues, MIVA Championship Tournament, and Division 2 Playdays.

All fees are payable by February 6, 2008 or such other time as approved in writing by MIVA Treasurer. Remit to:
