- Fee covers: MIVA League Dues, NCVF team registration, MIVA Championship Tournament, BTMVA Round Robin, BTMVA Championship, Conference Awards and conference administrative expenses. The cost of the BTMVA Awards Banquet will be billed separately.

- Fee covers: MIVA League Dues, NCVF team registration, MIVA Championship Tournament, M10VC Playdays and Championship, and Conference Awards.

- Fee covers: MIVA League Dues, NCVF team registration, MIVA Championship Tournament, GMMVC Playdays and Championship, and Conference Awards.

- Fee covers: MIVA League Dues, NCVF team registration, MIVA Championship Tournament, and Division 2 Playdays.

All fees are payable by January 19, 2010 or such other time as approved in writing by MIVA Treasurer. Remit to:
